Inconsistent documentation of oxygen flow rate may result in administration of an incorrect oxygen dose, leading to hypoxia, hypercapnia, worsening respiratory status, and increased risk of hospitalization.
Conflicting oxygen flow rate documentation may be cited as inaccurate assessment and inadequate ongoing monitoring.
Oxygen is a physician-ordered treatment, and discrepancies in documented flow rates may raise concerns regarding medication/treatment accuracy, patient safety, and the reliability of the clinical record. The issue was identified during QA review and corrected after clinician clarification to ensure consistency with the physician’s order.
